training

France’s training for family physicians includes 6 years of Medical School followed by 3-years Residency Program divided into 6 semesters and 200 hours of university training (personal interview, 2018). 37 UFR = Unite de Formation et de Recherche (“Training and Research Unit”), includes 2 UFR at Universitie de Lyon, 2 UFR at Universitie de Lilles and 7 UFR in Paris (personal interview, 2018)

practice

In France all family physicians have six generic skill requirements: (Les 6 Compétences Génériques | Département Médecine Générale - Université Paris 7 Diderot, n.d.)

  • First resort, uncertainty, urgency

  • Global approach considering the complexity

  • Education, prevention, screening, individual and community health

  • Continuity, follow-up, coordination of care around the patient

  • Relationship, communication, patient centered approach

  • Professionalism

In 2018, a nursing diploma of advanced practices was put in place, for the moment they are not trained (personal interview, 2018)

Family doctors in France are called “medicine generale”, which take care of patients from 0 to 99 years (or more), physicians provide pediatric follow-up, also gynecological (contraception), small gestures (IUD insertion, infiltration, suture, immobilization), followed by chronic diseases, first resort, and can also provide home visits (personal interview, 2018)

Family doctors act as gatekeepers for specialist care. Individuals can see specialists by themselves, however they are not reimbursed for the visit (personal interview). Some doctors work by themselves, but can work in teams, especially when doing research. Those who work by themselves, may be due to the limited number of doctors. Overall, a collaborative model (personal interview).
