The health system is organized in a pyramidal structure. Primary healthcare, such as clinics, and local hospitals represent the first resort for patients. It can be either private or public. The second tier is provincial public hospitals and private clinics, while the third is regional hospitals. The fourth tier is university hospitals, (WHO, 2006).

Training:

The ENSP has created one program in 2015 with 40 graduates as a degree designed to create recognition for a family physician in this country. Medical school lasts for 7 years. The seventh year is an internship and once "competency" has been achieved the student will defend his/or her thesis.

Practice:

Though the referral system exists in this country it faces several barriers that do prevent it from functioning properly. Patients meet their primary healthcare worker and receive a slip that refers them to a different hospital or health centre. These slips are not put on medical record and the patient is not tracked to ensure they meet the specialist and so the patient might miss their appointment and so their care is compromised.
